2. Purpose of this Manual

This manual provides an outline of the types of records held by KOHC and explains how one may submit a request for access to these records in terms of the PAIA Act.

3. Records Held by KOHC

We hold the following categories of records:

• Company Records: Financial, operational, and administrative records.
• Employee Records: Internal staff documentation.
• Client/Patient Records: Occupational health files, medical surveillance data, and Certificates of Fitness (governed by strict medical confidentiality and POPIA).

4. Request Procedure

To request access to a record, please use the prescribed Form 2 (Request for Access to Record of Private Body) as published by the Information Regulator of South Africa. Submit the completed form to our Information Officer at reception1@kimohc.co.za. Please note that access to medical records is strictly controlled and may require written consent from the patient.
